Employee enrollment fee: $50
Employee monthly fee: $17 or $34
If the employee utilizes the facility 8 or more times a month, then the rate is $17
If the employee utilizes the facility 7 or less times a month, then the rate is $34

Rates for Additional family members:
Spouse enrollment fee with contract: $25
Spouse enrollment fee without contract: $50
Spouse monthly fee with contract: $29
Spouse monthly fee without contract: $32

Child (age 13-24) enrollment fee: $25
Child (age 13-24) monthly fee: $22

Child 12 and under enrollment fee: $25
Child 12 and under monthly fee: $5
