Sometimes things happen that could cause a privacy or security problem for a patient. By reporting these incidents, you can help ensure Baptist fosters and maintains the safest culture possible.

The Privacy and Security Incident form makes reporting an incident easy and efficient.

When to use the Privacy and Security Incident form

You should use the Privacy and Security Incident form for reporting HIPAA-related events.

Reporting instances include:

  • Inappropriate disclosure of or access to patient information
  • Fax to the wrong location
  • Suspected medical identity theft
  • Data integrity (overlay, document in the wrong record)

The Privacy and Security Incident form does not replace the Electronic Occurrence Reporting (EOR) form or the Risk Man (RL Solutions) form team members use now. Those forms should be used to report patient safety-related events.

Both the Privacy and Security Incident form and the EOR form can be found on the MyBaptist Sharepoint site under the “Resources” tab. Click on the “Electronic Occurrence Report (EOR)” link to find links to both forms.

Central Mississippi team members can find the Privacy and Security Incident form and the Risk Man form on CareNet.

All privacy and security and patient safety incidents should be reported immediately.
