Keep reading for important details, including deadlines for getting a biometric screening or completing a BestHealth Tobacco Cessation Course.
Rewards for Working Toward a Healthier Lifestyle
Here’s how to receive up to $300 in rewards:
- Complete an annual wellness visit with a primary care provider — $100 gift card
- Have a body mass index (BMI) between 18.5 and 24.9 — $100 gift card
- Show a blood glucose of 99 or less or A1C of 5.6 or less — $50 gift card
- Demonstrate a blood pressure of 119/79 or less — $50 gift card
How to Receive Rewards and Avoid the Tobacco Surcharge
Complete the Health Risk Assessment Questionnaire on the inHealth Wellbeing app or web portal and make an appointment with your primary care provider for your biometric screening between July 1 and Nov. 13, 2026. Your biometric screening includes a tobacco/nicotine-status test (primary holder only; not spouse or dependents). A tobacco/nicotine-only test will not be accepted.
If you test positive or self-attest for tobacco/nicotine and want to avoid the $100 monthly tobacco surcharge, you must complete the virtual five-week BestHealth Tobacco Cessation Course between July 6 and Nov. 16, 2026. The BlueCross BlueShield online tobacco cessation course is NOT an option.
