Hopefully by now you’ve had a chance to go to Humana.com and take, or retake since Jan. 1, your online Health Assessment. Congratulations! That’s the first step. If you haven’t had a chance, please do so now. It only takes a few minutes. Check out the Baptist Leader article titled, “HumanaVitality: Getting to Silver” for additional details.
The next step to get to Silver is to go get a Vitality Check. A Vitality Check is a health screening that measures your body mass index, blood pressure, cholesterol and blood sugar levels. These are important measurements that help you understand where your health stands today and where you may want to work on making some improvements.
You get credit for this screening each year, so you’ll need to have this screening after Jan. 15 to count during this year’s program. So how do you get a Vitality Check?
The best option is to set an appointment with your primary care physician and get it taken care of during your annual screening. You can simply take the PCP check form form with you and have your doctor fill in the needed information. Don’t currently have a physician? Don’t worry, you can go to baptistdoctors.org or Aetna’s website and search for a physician in-network. (To get to Aetna’s physician search page, go to MyBaptist and hover over the “Resources” tab at the top of the page. From there, click on the “Benefits” link, then the “Health Insure Plans” link. Scroll down to the “Links” section of the page and click the “Aetna Doc Find” link.)
Having a primary care physician is important. They can give you an in-depth understanding of your health and advise you on how to reach your goals of a healthier life. Once you have the completed form in hand, you can submit the results in a few different ways.
- Fax it in to HumanaVitality at 1-877-250-7814
- Mail it to HumanaVitality, P.O. Box 14613, Lexington, KY 40512-4613
- Scan it and upload it through the Humana website (www.humana.com). Just click the “My Messages” link in the upper right hand corner of your personal Vitality page, attach the scanned form to an email, then click “Submit.”
So, how many Vitality points can you get for a Vitality Check? How does up to 4,000 points sound?!
You will receive 2,000 Vitality points just for getting the screening completed. If you have measurements in a healthy range, you can earn up to an additional 2,000 as well. See the chart below to find how many points you can earn for each and what the healthy ranges are.
Don’t fall into a healthy range? No problem. You will have a chance to earn even MORE points by working on and achieving GOALS that are recommended to you for any measurements that are not in a healthy range. We’ll have more information on GOALS in future Leader editions. But right now, get your Vitality Check and follow the Leader for your next step toward Silver and beyond.
